What is Bridge Point Community Services?

 Bridge Point Community Services, Inc. is a nonprofit organization dedicated to helping individuals and families overcome barriers to stability and independence. We provide person-centered services, including case management, transportation assistance, meal delivery for seniors, Organization Representative Payee services, and community resource navigation. 

Who do you serve?

 We serve eligible individuals within our service area, including:

  • Seniors 
  • Adults with disabilities 
  • Individuals experiencing transportation barriers 
  • Individuals and families facing financial or housing challenges 
  • People needing assistance accessing community resources 
  • Individuals who require Representative Payee services 

Eligibility may vary depending on the program and available funding.

What areas do you serve?

 Bridge Point Community Services provides services within the Piedmont Triad area. Please contact us to confirm whether you are eligible based on your location. 

Are your services free?

 Many of our programs and services are provided at no cost to eligible individuals through community partnerships, grants, and charitable support. Some services may have specific eligibility requirements or funding limitations, and availability may vary by program.

While there is no obligation to give, donations are always appreciated and help Bridge Point Community Services expand programs and continue serving individuals and families in our community.

Services

What is case management?

Case management is a personalized service that helps individuals identify their needs, set goals, and connect with community resources. Our case managers work alongside clients to develop practical solutions that support long-term stability and independence. 

What transportation services do you provide?

Transportation may be available for:

  • Medical appointments 
  • Pharmacy visits 
  • Grocery shopping 
  • Social Services appointments 
  • Other essential community services 

Transportation is subject to eligibility, scheduling, driver availability, and service area.

Do you provide emergency transportation?

No. We do not provide emergency or ambulance transportation. If you are experiencing a medical emergency, call 911 immediately. 

What are Organization Representative Payee Services?

As an organizational Representative Payee, Bridge Point Community Services may help eligible individuals manage their Social Security or SSI benefits. This includes paying essential bills, budgeting funds, maintaining financial records, and helping ensure benefits are used to meet the beneficiary's needs. 

Do you deliver meals?

Yes. We offer weekly prepared meal delivery for eligible seniors within our service area, subject to program availability and capacity. 

Can you help me find housing?

While we do not provide housing directly, we assist individuals by connecting them with housing resources, rental assistance programs, and community organizations that may be able to help. 

Can you help me apply for benefits?

Yes. We assist individuals with accessing community resources and may provide guidance or referrals related to available public benefit programs, depending on individual needs. 

Eligibility & Referrals

How do I know if I qualify for services?

Eligibility varies by program. Our staff will complete a brief intake to determine which services may be available based on your needs, location, and program requirements. 

Can a family member refer someone?

Yes. Family members, caregivers, friends, and community members may contact us on behalf of someone who may need assistance. .

Do you accept professional referrals?

 Yes. We welcome referrals from:

  • Healthcare providers 
  • Hospitals 
  • Social workers 
  • Behavioral health agencies 
  • Senior centers 
  • Faith-based organizations 
  • Community agencies 
  • Other nonprofit organizations

How do I request services?

You can contact Bridge Point Community Services by phone, email, or through our website. Our team will discuss your needs, determine eligibility, and explain the next steps 

What should I have available when I contact you?

Depending on the service you're requesting, it may be helpful to have:

  • Emergency contact information 
  • Basic information about your needs 
  • Any relevant benefit or insurance information (if applicable) 

Our staff will let you know if additional documentation is needed.

How long does it take to begin services?

Service availability varies depending on the program, eligibility, funding, and scheduling. We strive to respond to all inquiries promptly and will keep you informed throughout the process.

Do I have to be affiliated with a church or religion to receive services?

No. Bridge Point Community Services serves individuals of all backgrounds, cultures, and beliefs. While our organization is guided by biblical principles of compassion, integrity, and service, participation in our programs is never contingent upon faith, religious affiliation, or participation in religious activities.

Will my personal information be kept confidential?

Yes. We respect your privacy and are committed to maintaining the confidentiality of your personal information in accordance with applicable laws, regulations, and organizational policies. Information is shared only when necessary to provide services or when required by law. 

How will I be treated when I receive services?

Every individual is treated with dignity, respect, and compassion. We are committed to providing person-centered services that recognize each person's unique strengths, goals, and circumstances. 

Can I receive more than one service?

Yes. Depending on your needs and eligibility, you may qualify for multiple programs or services. Our team will work with you to determine the most appropriate support and develop a plan tailored to your situation.

What if Bridge Point Community Services can't provide the service I need?

If we are unable to meet a specific need directly, we will do our best to connect you with trusted community partners and resources that may be able to assist.
